15 pro max in white. Has always had a screen protector and case on - however… I dropped it and it hit the corner of the chair just right causing the screen to crack underneath of the protector. The screen works perfectly still, no glitching or dead spots. Comes with 5 cases a the charger. I used it on Verizon but it is paid off and unlocked.
