@melreffell Hi! I believe this jacket is true to size for this designer.(Italian size 40) It was not altered. Might be a little smaller though compared to US size 4 as I often wear size 2 in other makes. The measurement between the shoulder seams across the back of the jacket is 14 and3/4 inches. Hope this helps!
