Dr. Martens 2976 Hi Smooth Leather Chelsea Boot. These are taller on the ankle than the classic chelsea boots.
Size 7, but fit more like 7.5.
Photos 4-5 show the deep creases by the toes (not through to the lining), but there is still lots of life left!
