Shrine With Child Jesus -1941

A statue looking like a home shrine of a young Jesus giving out the Eucharist with a chalice, presumably with wine, in his other hand. He is standing atop clouds while rays of light emanate from him. There is an inscription in French on the back which is difficult to read. I believe the 1st line is "to and from." The second line says that it was commissioned on the 13th of April 1941 but I am unable to decipher the last line. Do note that there are 2 chips which I included close up pictures of. It is about 8" tall, 5" wide and 3 1/2" deep. I believe that its made of plaster.
