This is a vintage macrame rope belt with gold tone heart medallions, typically considered a boho, western or hippie style accessory. These types of braided or knotted cord belts with metallic accents were very popular in the 1980s and 1990s fashion. It is stretchy and adjustable by opening the gold tone hinges.
